Online complaint form

This page is designed to help you determine whether NSW Ombudsman is the right agency for you to approach in your bid to try to resolve your complaint. If you are certain your complaint falls within the jurisdiction of the NSW Ombudsman, you may go directly to the bottom of this page and answer the question 'Have you already made a complaint to the agency?" If you click "yes', you will be taken straight to the complaint form; if you click 'no', you will be taken to the 'Tips for making a complaint' page where you will find more information about how to make a complaint.

Please take a few minutes to read the following information.

The NSW Ombudsman deals with complaints about NSW agencies or their employees. These agencies include government departments, statutory authorities and some non-government agencies.

There are other complaint handling bodies that deal with various problems you may have with a government or non-government agency.

If you have a complaint about:

  • a doctor, health care provider, hospital or health service click here for the Health Care Complaints Commission
  • an insurance company click here for the Financial Ombudsman Service

  • a telecommunication company or internet provider click here for the Telecommunications Industry Ombudsman

You can complain to us if you have made a complaint to a NSW agency and are dissatisfied with the way it was handled. We deal with complaints about NSW agencies such as government departments, councils, correctional centres, juvenile justice centres, police, public schools, universities, TAFE colleges, area health services, child care centres, community services providers and organisations that provide residential care services.
